Home Care Worker

Job Description: 
The core value of New Directions Dementia Home Care Service is to provide high quality and competitive domiciliary care to people with dementia. Working as a member of the Dementia Home Care Service and in accordance with an agreed care plan provide personal care, domestic and social support to people with dementia in their own homes to enable them to maintain their ability to live independently within the community.

Major Tasks:

1. To support service users to accomplish tasks relating to the maintenance of personal hygiene, physical health and mobility within their homes

2. To provide domestic support

3. To provide social support to service users liaising with other members of the family and community care team as necessary

4. To assist service users maintain good standards of nutrition

5. To work as a member of the Dementia Home Care Service maintaining contact with others and attending meetings and training as required

Duties Typically Include:

6. To support service users to accomplish those tasks relating to the maintenance of personal hygiene, physical health and mobility within their homes.

7. To respect the rights of service users, and to strive to establish and maintain their trust and confidence, and that of their families and personal carers.

8. Assist service users to wash, bath or shower, shave, maintain dental hygiene, access the lavatory or use equipment relating to continence, maintain their preferred standard of grooming and dress and undress.

9. Assist service users who require support to maintain mobility within their homes using any equipment provided to enable safe transfers and enabling access to transport to other services.

10. Support service users to use equipment and adaptations provided to assist them in daily living or with rehabilitative activities.

11. Support / prompt service users to take medication and maintain records as required.

12. Support service users who require assistance to eat or maintain a prescribed diet

13. Observe signs of physical or mental disorder and bring these observations to the attention of the appropriate person

14. To provide domestic support

15. Assist service users with essential laundry and ironing or to access laundry facilities

16. Light fires and ensure that adequate fuel supplies are available for the service user

17. Assist service users to clean rooms in daily use using equipment and materials provided by the householder

18. Maintaining an hygienic environment

19. To provide social support to service users liaising with other members of the community care team as necessary

20. Collect pensions and other allowances and assist in the paying of bills and maintain records of financial transactions undertaken

21. Collect prescriptions and maintain appropriate records

22. Monitor service users' well-being, encouraging and supporting them to seek medical help if necessary and maintain positive relationships with service users' families, friends and neighbours

23. Encourage and assist service users to participate in social activities and receive visitors

24. Ensure service users' homes are secure in accordance with their wishes.

25. Ensure that service users are made aware of any changes to their care arrangements and the reasons for them as soon as possible

26. To be aware of service user’s spiritual needs and by liaison with the service user, their relatives, friends and advisors, ensure that these needs are met to the service user’s satisfaction.

27. To be aware of the service users need to develop or maintain social activities and individual social networks.

28. To assist clients maintain good standards of nutrition

29. Encourage service users to maintain good standards of nutrition, assisting with the preparation of light meals and snacks if required and enabling them to access main meal delivery services including meals on wheels

30. Assist service users to maintain good standards of food hygiene

31. To work as a member of the Home Care Team, maintaining contact with others and attending meetings and training as required

32. To maintain regular contact with all members of the Community Services team, attending team meetings and training as required.

33. Ensure other members of the Home Care team are made aware of service users changing needs, seeking advice and guidance as necessary.

34. To ensure that your timesheet is an accurate reflection of your work and to be responsible for ensuring it is returned to the office at the specified time.

35. To be accountable for your work practice and take responsibility for maintaining and improving your knowledge and skills.

36. To undertake training as necessary to perform your duties to the highest standards, including achieving NVQ2 in care within 12 months of commencement of employment.

37. Attend supervision meetings with Senior Home Care Worker as required.

38. To undertake any other reasonable duties as required by the service.
